After that one year of internship and four years of residency, you can subspecialize into multiple different subspecialties, all the way from pediatrics to rhinology to laryngology to plastics to head and neck cancer or otology. I subspecialized in head and neck cancer and that's where you do free flaps to do cancer reconstruction of the face. It's very involved and usually best to be done in a large city. I decided to do less than that and I am now in a small town but I love it. I can still do multiple range of head and neck cancers and it's a very rewarding profession.
